Scarless Lower Face and Neck SMAS Lift via Serdev Suture®
What is SMAS?
In 1972 it was demonstrated that after classic face lifting the skin is not stable and elongates in minutes. The Superficial Muscle-Aponeurotic System (SMAS) of the face was described by Mitz and Peyronie in 1976. Since then, lifting of SMAS has become the obligatory method for surgical face lift (including lower face and neck lift), in order to maintain a long lasting result. The skin and soft tissue are strongly fixed to it in the temporal area.
Separating the skin from the SMAS leads to elongation of the skin, which has lost its stable support, and should not be performed. Also, in a subsequent SMAS lifting the skin is not affected skin any more, because it is not well attached. The consequences of separating the skin from the SMAS are a mask-like stretched appearance, changed hairline position behind the ears etc.
When is Lower Face and Neck Lift necessary?
Ptosis and laxity of the "subdermal facial mask" (the SMAS) in lower face and neck reflect the signs of lower face aging. The primary goal for this procedure is straightening and repositioning of the subdermal mask (the SMAS/platysma). We thus achieve refreshing the facial appearance as well as obtain rejuvenation and beautification. The effect of the procedure should correspond to the aesthetic angles, proportions and anatomic position typical for youth. Lower face and neck lift is used for sculpturing of the jaw line, as well as softening of the nasolabial and marionette folds. It is mostly done in monor and medium jowling, and submental laxity. The procedure ideal in cases of beautification, associated with early or medium facial ptosis.
"What if I have subdermal fat?"
As subdermal and submental fat in the lower face and neck (causing square, "bulldog" appearance) is superficial to SMAS, it cannot be lifted with the SMAS lift. In such cases, for achieving the best possible lifting results, an ultrasonic liposculpture may be required to remove the fat depots prior to the lower face and neck lift.
Serdev Suture® Lift
The concept of Serdev Suture® lift in this case is to tighten and elevate the lower face and neck without unnecessary incitions. Since the skin and soft tissue are attached to SMAS, lifting and fixing the latter to a more favourable position enables repositioning of soft tissue structures as well.
This type of lifting can be applied at almost any age if there is sagging of the skin and wrinkling of the neck. It is also applicable to men because there is no cutting of the skin and no scars around the ears and the hair. The results are very satisfactory to patients and are durable in time. Of course, the aging process cannot be stopped, but it will continue from the new, higher, more youthful position.
The are no scars, only skin puncture points, which heal and disappear in 2-3 days. Patients can immediately return to their sicial life. There might be a tight, "pulling" sensation in the first week after surgery.
